Two cases of unilateral paralysis of the diaphragm in the newborn treated surgically.

چکیده
Unilateral paralysis of the diaphragm in the newborn infant is not a common condition. It does not appear to be well known in this country, for there have been no reports of the condition in the British literature. It is, however, a serious condition, and is often fatal because of respiratory difficulty and, to a lesser extent, of the feeding difficulties always associated with dyspnoea in infants. This contrasts riarkedly with the absence of any such respiratory difficulty from diaphragmatic-paralysis in adult life or later in childhood and is due to the very mobile mediastinum of the infant becoming markedly displaced, so that, in addition to the lung on the side of the paralysed diaphragm being compressed, the lung on the opposite side also becomes compressed. This is clearly seen in Figs. 1A and 4A. The condition has been reported most often on the right side. It may be associated with a brachial plexus injury of the Erb type or it may be an isolated lesion. Schifrin, writing in 1952, was able to collect from the literature nine cases of isolated paralysis of the diaphragm in the newborn, all from American and German sources, and he added two further cases. The following two cases of isolated diaphragmatic paralysis are, therefore, the twelfth and thirteenth to be reported. Incidentally, none of the previously reported cases have been treated surgically, nor, even when ending fatally, does operation appear to have been considered.

Background: Acquired paralysis of the diaphragm is a condition caused by trauma, surgical injuries, (lung cancer surgery, esophageal surgery, cardiac surgery, thoracic surgery), and is sometimes of an unknown etiology. It can lead to dyspnea and can affect ventilatory function and patients activity. Diaphragmatic plication is a treatment method which decreases inconsistent function of diaphragm...
متن کاملEVENTRATION OF THE DIAPHRAGM: A 10- YEAR EXPERIENCE
Eventration of the diaphragm is a condition characterized by an attenuated but otherwise intact diaphragm. Twenty patients (age range: 2 days to 11 years) were treated for eventration of the diaphragm during a ten year period (19 85-1995). There were 11 girls and 9 boys with a mean gestational age of 37 weeks. 14 patients were born with cephalic presentation, 3 patients with breech deliver...
متن کاملBedside lung ultrasound in emergency (BLUE) protocol: a suggestion to modify.
In the article by Lichtenstein et al (July 2008), 1 the authors describe the use of lung ultrasound in evaluating patients with acute respiratory failure. The algorithmic approach Bedside Lung Ultrasound in Emergency, the BLUE protocol, is described with a diagnostic accuracy of 90.5%. Three hundred and one consecutive patients with acute respiratory failure were assessed; 260 were included (ca...
متن کاملParalysis of the diaphragm after pneumonia and of undetermined cause.
Unilateral paralysis of the diaphragm not due to some obvious cause such as neoplasm or pulmonary tuberculosis has been said to be uncommon (Freedman, 1950). I present 16 such cases found during two years in a chest clinic. My purpose is to show that this is a comparatively common condition and that it may be caused by simple pulmonary inflammation (referred to in this paper as pneumonia). Clin...
متن کاملAssessment of diaphragm paralysis with oesophageal electromyography and unilateral magnetic phrenic nerve stimulation.
The purpose of this study was to establish a sensitive and reliable method of diagnosing diaphragm paralysis by recording the diaphragm compound muscle action potential (CMAP) using a multipair oesophageal electrode and unilateral magnetic phrenic nerve stimulation. An oesophageal electrode catheter was designed containing six coils (1 cm wide and 3 cm apart), creating an array of four sequenti...
